Every Loss, No Total
A system can record every loss accurately and still never recognize the total that would require an investigation.

Your bank statement can reveal an erroneous charge. It cannot reveal how many other customers received it.
A rule requiring investigation above a certain sum must also specify which losses may be counted together.
Suppose a regulator must investigate losses above $100,000. One dollar wrongly taken from each of a million accounts crosses that threshold if losses from the same billing error count together. If the rule counts each account separately, no case qualifies. The difference is not an error in the arithmetic. It is a choice about which total has legal force.
Consider a faulty component used in several products. Failures arrive as repair requests under different brands, in different places, at different times. Linking them to a common defect may establish losses large enough to require investigation. The damage did not grow when the records were joined. It became possible to count the losses together.
A rule may forbid combining related losses even when their total is known. Or it may permit combining them while the evidence needed to establish their connection remains scattered beyond the reach of those harmed.
Each customer holds one invoice; each workshop sees a few broken parts. An invoice may document a repair cost without identifying the internal component that failed. The manufacturer may possess the records needed to connect the failures—and face investigation if the resulting total crosses the threshold. Concealment is unnecessary. Records designed to process individual complaints can work perfectly without supplying the links needed to establish a collective loss.
A large sum is not proof of a common cause. Similar failures can have different origins; losses need a defensible connection before they can be counted together. But difficulty establishing that connection is not evidence that the damage is small.
Your institutions can record every loss accurately and still never recognize the total that would require them to investigate.
